LAPAROSCOPIC SURGERY
WHAT IS LAPAROSCOPIC SURGERY?
Laparoscopic surgery is also called Keyhole surgery or Minimally Invasive Surgery or Minimal Access Surgery. Usually, the term laparoscopic surgery is reserved for keyhole surgery or minimal access surgery done in the abdomen and the term Thoracoscopy is used for the minimal access surgery performed in the chest.
In laparoscopic surgery, surgery is performed by making small holes or small incisions in the abdomen through which a telescope and various instruments are introduced and surgery is done through these holes. In open surgery, a big incision is made to perform the same surgery.
HOW LAPAROSCOPY SURGERY IS PERFORMED?
Laparoscopic surgery is usually carried out under General Anaesthesia.
During this operation, 2-4 abdominal incisions between 0.5 and 1 cm in length are made in the patient. The abdomen is inflated with gas (usually carbon dioxide) and distended using a small needle and then the other small incisions or holes are made through which small tubes called Ports are introduced and secured and through these holes, the Telescope and the various instruments are introduced and surgery is performed.
Inflating the belly provides the surgeon with a better view of internal organs and more space for them to operate.
The telescope is connected to the camera head which connects to the monitor and the image is displayed on the monitor with manifold magnification.
A fibre optic light source cable is connected to the telescope which acts as a source of light. The surgeon sees the images on the monitor and operates with the laparoscopic instruments.
The laparoscopy and other tools are taken out when the procedure is finished, the gas is let out from the abdomen and the incisions are then closed.
WHAT PROCEDURE CAN BE DONE THROUGH LAPAROSCOPY?
IS LAPAROSCOPIC SURGERY SAFE?
Compared to open surgery, laparoscopic surgery is safer, as it reduces the time spent by the patient in the hospital or on bed rest, less pain and the occurrence of incisional hernia is low. Laparoscopy offers a magnified image giving more clarity and safety.
BENEFITS OF LAPAROSCOPIC SURGERY
- 1.Smaller scar
- 2. Lesser post-operative pain
- 3. Quick recovery
- 4. Quick return to normal activities
- 5. Reduction in wound infection
- 6. Avoids larger incision
- 7. Minimal risk of incisional hernia
- 8. Improved visualization during surgery
- 9. Lesser duration of surgery
- 10. Lesser duration of stay in hospital
LIMITATIONS OF LAPAROSCOPIC SURGERY
- 1. The small incision provides the surgeon with a limited range of motion.
- 2.The surgeon can also have trouble touching the tissue . This makes it difficult for surgeons to determine how much force should be used. The tactile sensation of touching the tissue with the hand during surgery to make decisions and guide in surgery is not available in laparoscopic surgery.
- 3. Extraction of large specimens can’t be done through laparoscopy.
- 4. Some underlying medical issues prevent the patients from receiving general anaesthesia, which is often required for laparoscopic surgery.
- 5. Haemostasis is difficult.
- 6. Reliance on remote vision and operating
RELATIVE CONTRAINDICATION FOR LAPAROSCOPIC SURGERY
- 1. Increased intra-abdominal pressure
- 2. Multiple previous surgeries
- 3. Ascites
- 4. Extensive abdominal adhesions
- 5. Patients who can’t tolerate general anaesthesia
- 6. Clotting abnormalities
- 7. Decreased venous return
- 8. Acute heamorrhage
- 9. Uncontrolled Shock
- 10. CO2 retention or hypoventilation
- 11. Portal hypertension
